Welsh nationalist party Plaid Cymru is second with 27% in the survey on which party voters would back for the Senedd. , which has had more than 100 years of electoral success in Wales, trails behind in third place on 18%.
The Tories are fourth on 11% in the poll of 2,101 adults in Wales carried out from June 5-16.
Meanwhile, the Liberal Democrats and the Greens both take 7%.
The poll comes ahead of the next Senedd election which will take place in May 2026.
It comes as Reform is riding high in national opinion polls following its success in last month's local elections.
The party won hundreds of councillors and took control of 10 town halls.
Reform also won its fifth MP in the Runcorn and Helsby by-election, along with two mayors.
It comes as the latest YouGov Westminster voting intention poll put Mr Farage's party in first place on 27%.
